Stepping Stones Primary Care will permanently close on Friday, August 28, 2026. This will be our final day of providing patient care. After that date, we will no longer be available to provide medical treatment, routine prescription refills, respond to medical questions, or schedule appointments.

Your health and well-being remain our top priority. To help ensure continuity of your care, we strongly encourage you to establish care with a new primary care provider as soon as possible, particularly if you have chronic medical conditions or require ongoing prescription medications.

Prescription Refills

Prescription refill requests received on or before Friday, August 28, 2026, will be reviewed and processed when medically appropriate. Requests received after that date cannot be honored and should be directed to your new healthcare provider. If you anticipate needing medication refills before establishing care with another provider, please contact our office as soon as possible.

How to Access Your Medical Records

• You or your new healthcare provider may request a copy of your medical records by completing a HIPAA-compliant Authorization for Release of Medical Records. Please visit www.steppingstonesprimarycare.com or call 252-991-7009 to obtain the form.
• Medical records will be securely maintained in accordance with North Carolina law.

Finding a New Provider

• Contact your health insurance company or use its online provider directory to locate in-network primary care providers.
• If you need assistance identifying another primary care provider, our staff will gladly assist you in locating providers accepting new patients. However, we cannot guarantee provider availability or insurance participation.

Medical Emergencies

If you experience a medical emergency, call 911 or go immediately to the nearest emergency department. Do not delay emergency care by attempting to contact our office.

Any appointments scheduled after Friday, August 28, 2026, are cancelled due to the permanent closure of the practice. Our office staff will remain available through Friday, August 28, 2026, Monday through Friday, 8:30 a.m. to 4:30 p.m., to answer administrative questions and assist with medical records requests.
